Job Title: Registered Nurse,
St. Paul's Towers
Department: Skilled Nursing (SNF)
Reports To: Director of Nursing
FLSA Status: Non Exempt
Primary Purpose: The RN acts as a role-model and demonstrates
professional behaviors in the supervision of staff caregivers,
consistent with Title 22 and Title 42 (OBRA) regulatory requirements.
The Registered Nurse makes regular rounds on the nursing floors and
provides leadership and clinical intervention as indicated, within
his/her licensure scope of practice and company standards.

ESSENTIAL FUNCTIONS:
- Ensures quality nursing care to all SNF residents
- Provide outstanding customer service to residents and guests
- Supports the SPT Mission, practices the Health & Wellness
Philosophy, nursing objectives and standards
- Participates quarterly in Performance Improvement Committee
- Receives and gives report at beginning and end of shift to
licensed staff and nursing assistants
- Observes, assesses, and evaluates resident’s condition,
communicates findings to physician and follows up changes in the
care plan and regimen. Note physician orders accurately and execute
them. Notify resident families or responsible parties of resident
changes of condition or needs
- Perform daily twenty-four (24) hour check of all SNF medical
records, reviewing changes of condition; patient, responsible party
and physician notifications; new orders; and care plan updates of
the new orders.
- Accurately documents in the resident’s medical record
observations, assessments and progress notes to reflect resident’s
regimen. Assures that Care Plans are current.
- Participates as a member of the interdisciplinary team (IDT) in
the assessment of the resident's care needs and the formulation and
updating of the care plans.
- Interpret physician orders and the care plan to residents and
their families providing information/instruction in a nonjudgmental,
compassionate manner
HUMAN RESOURCE DEVELOPMENT
- Role models professional behavior and coaches and mentors staff
to develop maximum potential
- Sets and achieves personal goals for professional growth
- Maintains current licensure and CPR
- Participates in SPT inservices and staff meetings
- Ensures staff satisfaction with recognition, retention and
recruitment programs
- Assures staff competency and performs staff evaluations as
indicated by COMPANY policy
- Recruits, orients and retains adequate levels of staff in each
category
SAFETY
- Assures that the SNF is a safe work and patient-care
environment.
- Adherence to Fire Life Safety codes/regulations
- Participates in Facility Emergency Plan training and fire
drills; and if an emergency or disaster occurs, immediately assumes
the pre-assigned emergency duties.
- Role models work tasks safely and in compliance with the
facility safety program
- Performs follow up investigations to all work injuries as
indicated by DNS
- Other duties as assigned by DNS
QUALIFICATIONS:
- Must possess a current California Registered Nurse
- Ability to read, speak well and be understood in the English
language
- Must have attention to detail, follow through and good
interpersonal/problem solving ability
- Strong medical surgical nursing skills
- Prior experience working in a skilled nursing facility highly
desirable.
- Must successfully pass a health screening including an approved
tuberculosis test
- Must successfully pass a Criminal Record Clearance through the
California Department of Justice and if reporting to work prior to
receipt of clearance certify in writing that they have never had a
criminal conviction relevant to retirement housing and health care
Preferred elements:
- Advanced degree in health care management and/or nursing
desired.
- Minimum of 5 years experience working as health manager
- Good understanding of long term care, Medicare, Title 22, OBRA
regulation
